The department uses an online web application for surveys:
http://biostat.mc.vanderbilt.edu/survey

Please contact ColeBeck for more information.

Viewing individual responses

  1. Log in and click "Manage my surveys"
  2. Click the "Stats" button for the desired survey
  3. Edit the URL: change "&cmd=home" to "&cmd=raw"
  4. If desired, copy the output into a spreadsheet and save as CSV
