Development and Implementation of a Predictive Model for Preemptive Pharmacogenetic Testing
One strategy for effective use of genomic information to individualize patient treatment is to store portions of the genomic information in the electronic health record (EHR) prior to clinical need. We will describe an implementation of such a pre-emptive genotyping program. Using EHR data, we built a model to predict the receipt of at least one of three medications (clopidogrel, warfarin and simvastatin) with documented pharmacogenetic effects. Model results were integrated within the electronic health record and risk of being prescribed a medication was calculated prior to scheduled patient appointments at a number of subspecialties. We report development of the model based on patients observed between 2005 and 2010, considerations for implementation of this model into a decision support system, and evaluation of the model for cohorts of patients who were observed at the same institution between 2011 and 2013.
