How To Complete Your Conflict of Interest Disclosure (COI)

As part of our annual performance evaluation requirements, all staff are required to complete a conflict of interest (COI) disclosure. This disclosure should be completed in conjunction with your annual performance evaluation.

To complete you COI:

1. Click on the following link to log into the system with your VUNET ID and Password: https://webapp-a.mis.vanderbilt.edu/coi/
   • If this link does not work, please copy and paste the link into your internet browser address bar.
2. Click Create Disclosure link
3. Follow the instructions, answering any required questions
4. Using the Continue button on the lower right hand side of the page, click through the disclosure pages, making sure that you have chosen an approver and entered their phone number in the appropriate field. Your approver is your primary supervisor who submits your annual evaluation.
   • Please remember that the function for choosing your approver works like the VU "People Finder." It works best if you enter only the last name of your approver, click the Select Approver button, and then click the Select button next to their name from the list which is returned.
5. If there are no blank answers or error messages on the last page of the disclosure process, you will see a Submit button on the right side of the page
   • You MUST click the Submit button in order for your disclosure to be submitted to your approver.
6. Notify your approver that you have submitted your disclosure for approval. The system does not currently have an email notification process so once you submit your disclosure, please tell your approver that you have submitted it for their approval.
   • Your approver should then go to the same website to review and approve your COI.
